Psychotherapy vs. Counselling in Ontario: What’s Covered, What’s Not?

Psychotherapy is a regulated, legally protected act in Ontario.
Counselling is not regulated by law, but still offered by trained professionals.

That means the kind of work each practitioner can legally offer—and the kind of language they can use—is different.

What Is Psychotherapy?

Psychotherapy involves treating diagnosed mental health disorders—like PTSD, anxiety, depression, or trauma that interferes with daily life.

In Ontario, only professionals who are licensed under certain regulatory colleges (like the CRPO, College of Social Workers, or College of Psychologists) are legally allowed to practice psychotherapy.

This work often includes:

  • Clinical assessments or diagnoses

  • Treatment of mental health symptoms

  • Techniques to resolve psychological disturbances

  • Regulated therapeutic language (e.g., “treatment,” “disorder,” “symptom management”)

What Is Counselling?

Counselling, on the other hand, is not a controlled act in Ontario. But that doesn’t mean it’s surface-level or fluffy.

Counsellors can support clients with things like:

  • Grief, loss, and life transitions

  • Relationship patterns and emotional processing

  • Sexual shame, confidence, and exploration

  • Identity work, self-esteem, and boundary-setting

  • Somatic awareness and nervous system regulation

What counselling doesn’t do is treat mental illness or use clinical language.
If someone is seeking care specifically for a diagnosed mental health condition, they should work with a regulated psychotherapist, psychologist, or social worker.

A Real-Life Example

A client says:

“I have PTSD, and it’s affecting my relationship.”

  • A psychotherapist might offer structured trauma treatment—like EMDR or CBT—focused on reducing symptoms.

  • A counsellor might support the client in exploring relationship triggers, shame, and communication—without directly treating the PTSD itself.

Both can help. The difference is in the scope, not the heart.

💸 What About Insurance Coverage?

Most extended health benefits in Ontario cover sessions with:

  • Registered Psychotherapists (RP)

  • Registered Social Workers (RSW/MSW)

  • Psychologists

  • Canadian Certified Counsellors (CCC)—only if your plan includes them

If you’re working with a either professional, you should always:

  1. Check your benefits booklet

  2. Ask your provider directly if your clinician’s title is covered

  3. Get clarity before booking if you need reimbursement

Why Does This Matter?

Because many people avoid support altogether—confused about what they can or can’t access.
